Rules and Expectations

Video clip participants: The 15 Seconds of Fame event is open to all current Sentry associates pursuant to the rules and expectations described in this document. Video clips can feature the submitting Sentry associate, a limited number of other Sentry associates, or the submitting associate’s immediate family members and pets living in their household. No other participants may be involved in the production of submitted video clips, and clips submitted involving other individuals may be disqualified or edited, at the discretion of the 15 Seconds of Fame committee. All participants must adhere to the Rules and Expectations.

Featured participants: The individuals featured in or involved in the production of submitted video clips must be limited to the following: (1) current Sentry associates or (2) the submitting associate's immediate family members and pets living in their household.

Video clip application and submission process: During the video clip submission period (described below), any current associate wishing to participate in the 15 Seconds of Fame event must submit the online registration form, the video clip being submitted, and an entry donation. The registration form and video clip must be submitted between August 31, 2026 and September 8, 2026. The committee will review the registration form and video clip to determine that it complies with these rules and expectations and Sentry's associate conduct policies. Approval of any registration form and its associated video clip for inclusion in the 15 Seconds of Fame event is at the sole discretion of the committee.

Submissions and donations: Approved video clips will be compiled by the event coordinators and posted to SentryNet. Once posted, Sentry associates will have an opportunity to donate to their favorite video clips, and clips receiving the highest donations for their respective post date will be entered in a finale. Sentry associates will have an opportunity to donate again to clips that didn’t initially make the finale to round out the Top 12. The finale will showcase the top videos from the preliminaries and second-chance voting periods. Associates may then donate to their favorite clips from the finale videos. Proceeds will be directed to a partner United Way organization associated with the donor’s Sentry office or the United Way of Portage County if no local partner United Way exists. There is no limit to the number of video submissions or donations. If a video submission is not approved or is disqualified by the 15 Seconds of Fame committee, the entry donation for that submission will be refunded to the associate.

Donations on daily preliminary video clips will be accepted from September 14 through September 18, 2026. Donate early and often on your favorite video. The two videos with the highest donation total for its posted date will make it to the finale.

Donations on the remaining video clips to round out the Top 12 will be accepted September 21 and 22, 2026. These donations will add to the original donation tallies for each video. If your favorite didn’t quite make the finale, now’s the time to rally votes.

Donations on finale video clips will be accepted September 23 through 25, 2026. Once we get to the finale, the donation tallies start all over again. If you have a favorite, get your donations in to nominate a 15 Seconds of Fame Champion!

Cost and submission: Please remit $5 per video clip application fee through this registration site. Upon payment, Swoogo will generate an email confirmation to associates with instructions and a link to upload their video. Registration forms and their associated video clips may be submitted beginning August 31, 2026 and must be received no later than September 8, 2026. Video clips which are not approved or which are disqualified will be eligible for an application donation refund.

Video clip time limits: Each approved video clip must be no longer than 15 seconds in length. Clips that exceed 15 seconds will either be clipped to the first 15 seconds or disqualified and not included in the daily releases of clips or the finale, at the 15 Seconds of Fame committee’s discretion.

Donations collected to select two additional videos to round out the Top 12

Voluntary participation: Participation in 15 Seconds of Fame is entirely voluntary, should be conducted on an associate's non-work time, and is unpaid. 15 Seconds of Fame participants assume full responsibility for any risks, injuries or damages which might be incurred as a result of their choice to participate in this event. Neither Sentry nor the United Way assume, and specifically disclaim, any responsibility for any such risks, injuries or damages. Neither the manner in which participants create video clips to submit, nor the number of video clip submissions or donations an associate chooses to make, is controlled by Sentry. Participation doesn't guarantee that any given video clip will be included in the event, chosen for the finale or voted among the top submissions for or the winner of the finale.

Conduct: The 15 Seconds of Fame event is intended to be workplace appropriate and professional, and participants are expected to conduct themselves in a manner consistent with such intent. Video clips involving unsafe, dangerous, risky or illegal activities won’t be accepted and will be disqualified by the committee. Participation is governed by these rules and Sentry’s associate conduct guidelines. Should you have any questions about whether any part of your video clip idea (including language, lyrics, dress, and the like) are appropriate, you should consult in advance with the 15 Seconds of Fame committee via jason.bach@sentry.com or lisa.walkush@sentry.com and err on the side of not including such content in your clip. Submitted video clips judged by the committee to be inappropriate or unprofessional will be disqualified at the committee’s sole discretion and not used in the 15 Seconds of Fame event.

Dress code: Submitted video clips that include inappropriate clothing or that are inconsistent with these rules and Sentry’s associate conduct guidelines will be disqualified at the committee’s sole discretion and will not be included in the daily releases of clips or the finale.

Release: Any participant appearing in a submitted video clip must consent and agree to our media release policy. Among other things, the release includes an agreement to allow Sentry to use participant images, sounds, and video footage for the benefit of 15 Seconds of Fame, Sentry and/or the United Way Campaign.

15 Seconds of Fame Committee: The 15 Seconds of Fame committee reserves the right, for any reason, to not accept or disqualify submitted video clips.
