I irrevocably consent to and authorize Sentry Insurance a Mutual Company, its subsidiaries and affiliates (collectively “Sentry”), and any others it authorizes, to use and reproduce my name, and/or any and all statements made by me, and/or photographs or video images that were taken of me or by me in connection with the Sentry Project listed below. Sentry may use such materials for any fundraising activity. I agree that all portraits, pictures, photographs, films, plates, negatives, reproductions, videotapes, films, verbal or written statements and digital files submitted by me shall remain the property of Sentry or of the United Way. I am over eighteen years of age. If any other persons appear in my photos or videos: (i) I am their parent or guardian and give my consent for them to appear; or (ii) the person(s) have given me permission to use their appearance(s) for this contest. If a location appears in the photo or video, it is my own property, a public place, or I received permission to use the location for this contest.
